- Venous insufficiency: Weak or damaged valves in the veins of the legs (as seen in varicose veins) make it difficult for blood to flow back to the heart, causing fluid to leak and pool.
- Kidney disease: Healthy kidneys filter waste products and excess fluid from the blood. When their function is impaired, fluid can build up in the body.

Given this wide range of potential causes, self-diagnosing oedema is unwise. If you experience new, persistent, or worsening swelling, a consultation with your general practitioner is essential.
The Conventional Approach: Medical Diagnosis and Treatment
When you present to a doctor with oedema, they will begin with a thorough history and physical examination. They will ask about when the swelling started, what makes it better or worse, and about your other symptoms and medical history. They will examine the swollen area, checking for pitting (where a finger press leaves a temporary indentation) and assessing the condition of your skin.
Depending on their initial findings, further tests may be required to pinpoint the cause. These can include:
- Blood tests to check kidney and liver function, protein levels, and markers of inflammation.
- Urine tests to look for protein loss, which can indicate kidney problems.
- An electrocardiogram (ECG) or echocardiogram to assess heart function.
- Ultrasound scans to check for blood clots (deep vein thrombosis) or problems with your veins and organs.
Treatment is always directed at the underlying cause. If your oedema is due to heart failure, for example, the focus will be on managing your heart condition. If a medication is the culprit, your doctor may be able to prescribe an alternative.

What the research shows
The scientific investigation into whole-body cold therapy is an evolving field. While much of the initial focus has been on athletes and recovery, some findings offer cautious insights into the physiological mechanisms that could be relevant to managing symptoms associated with swelling and inflammation. It is vital to interpret these findings with care, recognising the limitations of the current body of evidence.
A systematic review focusing on cryotherapy in inflammatory rheumatic diseases, which are often characterised by painful, swollen joints, concluded that cold applications can significantly reduce pain [2]. While the review primarily looked at localised cold and found mixed results for inflammation markers, the strong analgesic (pain-relieving) effect is a consistent finding. For someone whose oedema is linked to an inflammatory condition, this pain reduction could improve mobility, which in turn helps to pump fluid from the extremities.
Research on athletes provides further clues. Studies have examined how whole-body cold therapy affects markers of inflammation and muscle damage after strenuous exercise. One review noted that exposure to cold is thought to reduce the inflammatory response, a key component of the swelling that follows tissue micro-trauma [7]. Another, more recent review, highlights the complexity of these interactions, suggesting that cryotherapy may influence a range of cytokines, which are proteins that signal and regulate inflammation [6]. The findings cannot be directly extrapolated to say that cold therapy will resolve oedema from, for instance, kidney disease or venous insufficiency. The evidence base is, as one critical appraisal describes it, empirically based but still requires more robust, large-scale trials to confirm the theoretical benefits [5].

Because a primary cause of chronic oedema can be serious cardiovascular or renal disease, it is particularly important to highlight these contraindications. You should not use whole-body cold therapy if you have any of the following conditions, among others:
- Uncontrolled high blood pressure
- Congestive heart failure

- Severe venous insufficiency or history of blood clots (DVT)
- Severe kidney or liver disease
- Raynaud's syndrome
- Open wounds or skin infections
